Pappadoms, a takeaway at 193c Milton Road, Weston-Super-Mare was given the score after assessment on July 28, the Food Standards Agency's website shows.
It means that of North Somerset's 125 takeaways with ratings, 63 (50%) have ratings of five and none have zero ratings.
