Winscombe Fish Bar, a takeaway at 1 Sandford Road, Winscombe, North Somerset was given the score after assessment on April 6, the Food Standards Agency's website shows.
It means that of North Somerset's 129 takeaways with ratings, 65 (50%) have ratings of five and just one has a zero rating.
