Whitchurch Grill, a takeaway at 44 Belland Drive, Whitchurch, Bristol was given the score after assessment on September 11, the Food Standards Agency's website shows.
It means that of Bristol's 409 takeaways with ratings, 245 (60%) have ratings of five and just two have zero ratings.
