Golden Kitchen, a takeaway at 489 Whitehall Road, Redfield, Bristol was given the score after assessment on October 1, the Food Standards Agency's website shows.
It means that of Bristol's 401 takeaways with ratings, 246 (61%) have ratings of five and just one has a zero rating.
