Shang Hai Food Takeaway, a takeaway at 4 Windmill Hill, Bedminster, Bristol was given the score after assessment on September 7, the Food Standards Agency's website shows.
It means that of Bristol's 403 takeaways with ratings, 246 (61%) have ratings of five and just one has a zero rating.
