Hennesseys, a restaurant, cafe or canteen at 18 North Street, Bedminster, Bristol was given the score after assessment on April 27, the Food Standards Agency's website shows.
It means that of Bristol's 1,121 restaurants, cafes and canteens with ratings, 834 (74%) have ratings of five and just three have zero ratings.
