Coffee Lab, a restaurant, cafe or canteen at 22 Gloucester Road, Bishopston, Bristol was given the score after assessment on September 23, the Food Standards Agency's website shows.
It means that of Bristol's 1,130 restaurants, cafes and canteens with ratings, 849 (75%) have ratings of five and just one has a zero rating.
