Oak Tree Cafe, a restaurant, cafe or canteen at 308 Gloucester Road, Horfield, Bristol was given the score after assessment on March 28, the Food Standards Agency's website shows.
It means that of Bristol's 1,080 restaurants, cafes and canteens with ratings, 780 (72%) have ratings of five and just three have zero ratings.
