Miss Millie's Fried Chicken, a takeaway at 129 High Street, Staple Hill, South Gloucestershire was given the score after assessment on August 24, the Food Standards Agency's website shows.
It means that of South Gloucestershire's 138 takeaways with ratings, 88 (64%) have ratings of five and none have zero ratings.
